Please do not panic updating firmware etc. This is a very hard issue to exploit. And as long as there is not security issues towards your internet accessible software you use dont worry.
If you panic upgrade you can expect performance degeneration of up to 40% I've seen and heard of this is several setups.

It could be both without doubt. I don't know what kind of hypervisor they use or if they have just taken in to many clients on the node he is on. Endless scenarios :-).
Either way switching away from pbzip2 will definitely solve it.
@Nuxwin - True, but it's a VM it's hard to say how the underlying system is doing, by seeing if he has blocked IO's you get a pretty good picture.
Just for the fun of it, what does the apache log say when you try to update and it fails?

If they have compromised your system and gained root access, you should reinstall your server and recover from backup. Simple as that.
They could have placed a script god knows where, ps -ef and see if you see any abnormal processes, still, reinstall and recover from backup is without doubt the fastest way to recover a functional operation.
